What We’re Usually Taught About Autoimmunity

Most people—patients and physicians alike—are taught a simple story about autoimmunity:

Once it starts, it doesn’t stop.

The immune system “loses tolerance,” begins attacking the body, and from that point forward the role of medicine is to replace what’s lost or suppress immune activity.

That framework shapes how we think about conditions like autoimmune thyroid disease, type 1 diabetes, and multiple sclerosis. And to be fair, it’s largely based on patients diagnosed late, after real tissue damage has already occurred.

What’s discussed far less is an important nuance: while immune memory may persist, autoimmune activity—especially antibody production—can sometimes quiet or reverse early, if the triggers driving it are identified and reduced.

This patient came to me because of her joints.

She had inflammatory joint pain — stiffness, swelling, pain that interfered with daily life — but it didn’t behave the way classic autoimmune arthritis usually does. Her blood work was seronegative, meaning the common autoimmune markers we test for (like rheumatoid factor or anti-CCP antibodies) were negative. That doesn’t mean the inflammation wasn’t real. It was. It just didn’t fit neatly into a textbook category.

What she didn’t have were gastrointestinal symptoms.
No diarrhea.
No abdominal pain.
No weight loss.
No changes in bowel habits.
